Is a Lost Tooth a Lost Cause?

We’re used to seeing athletes wearing mouthguards at practice or play, because dental trauma is one of the most common (and predictable) sports injuries. But it’s not just athletes who are at risk, and there are some events in our daily lives that we just can’t predict. Car accidents, falls, workplace injuries, even innocent playground structures can take their toll on our smiles.

A major chip or a crack in your tooth is upsetting enough, and should be seen by Dr. Kelly Peterson as soon as possible. It’s even more unnerving when a tooth is knocked out completely. The technical term for a tooth which has been knocked out is an avulsed tooth, and it is a true dental emergency.

If you should suffer a partially or completely dislodged tooth, there is a possibility that your tooth can be reimplanted—if the damage isn’t too severe and if you get to our office immediately.

How can a lost tooth be saved? This is possible because of the complex biological engineering that anchors our teeth within the jaw. The root of a tooth is surrounded by the periodontal ligament. This connective tissue attaches the tooth to the alveolar bone of the jaw. When a tooth is knocked out, this ligament splits apart, leaving some tissue on the tooth root and some within the tooth’s socket.

To successfully reimplant a tooth, the connective tissue cells around the root of the tooth need to be vital, so that they can begin to reattach to the connective tissue left in the socket. Over time, this reattachment is complete, and the tooth becomes firmly anchored to the bone again.

It’s important to protect your tooth before you see Dr. Kelly Peterson to make sure there will be enough healthy tissue for reattachment. First of all,

  • Don’t panic! If you or a friend or family member lose a tooth, call your dentist or your emergency health care provider as soon as possible. You will get specific instructions for your specific situation.

If you are unable to reach a healthcare provider immediately, there are some general rules for taking care of an avulsed tooth:

  • Find the lost tooth. Don’t touch the root—use the crown, or top part of the tooth, to pick it up. You are trying to preserve and protect the connective tissue on the root surface.

 

  • If the tooth is dirty, gently rinse it in milk, saliva, or water. Don’t wipe it off, though. You could damage those connective tissue cells mentioned above.

 

  • Place the tooth back in the socket, if possible. Gently hold it in place with your fingers or bite down (again, gently). You can also place the tooth in your mouth next to your cheek.

 

  • If returning the tooth to the socket is not an option, or if you are worried about a child possibly swallowing the tooth, keep the tooth moist. Whole milk or solutions sold just for the purpose of preserving an avulsed tooth are better choices than water, which damage the tissue cells on the root. And never wrap the tooth tightly—this can also damage the connective tissue.

Above all,

  • Don’t delay! The faster a tooth is reimplanted in its socket, the greater chance you have of keeping it. Really, every minute counts. Reimplantations are more successful if they take place within 30 minutes. After an hour out of the mouth, your tooth’s chances of successful reintegration are lower—but still worth pursuing!

What will Dr. Kelly Peterson do?

  • Evaluate the avulsed tooth.

There are variables which can affect whether or not a lost tooth is a good candidate for reimplantation. Trying to replace a baby tooth, for example, could interfere with the formation of the adult tooth. An adult tooth that is broken will probably require a different type of treatment.

  • Prevent infection.

You might be given antibiotics and a referral to your medical doctor for a tetanus booster if needed.

  • Clean the site.

The socket will be gently irrigated to clean the area and to remove any clots that may have formed which can interfere with the tooth’s placement.

  • Recommend or perform a root canal.

Nerves and blood vessels within the tooth’s pulp generally don’t recover after a serious traumatic injury, so a root canal procedure could be necessary to preserve the health of your tooth. This procedure might be done immediately, or might be recommended for a later date.

  • Stabilize your tooth.

The tooth must be stabilized after being reimplanted, so Dr. Kelly Peterson will use a splint to anchor the tooth to the teeth next to it. The splint can be flexible or rigid, depending on the condition of the alveolar bone. Splinting generally takes from two to eight weeks, and you will be given detailed instructions for taking care of the area while you heal.

Losing a tooth is an alarming experience. But with prompt action, and a trip to our Marysville, WA office, it might be possible to make that loss only a temporary one.

What are dental implants?

Do you have a space where a tooth used to be? Were you born with a missing tooth? Are you getting ready for dentures? You may be a good candidate for a dental implant. Metal dental implants were invented in 1965. Technology continues to advance with millions of implants placed in the United States and Canada. Placing implants has become mainstream and a common practice for offices like ours.

A dental implant is a small titanium post, which resembles a screw with threads. The post also has holes for bone to integrate. A dental implant is placed into the jawbone during a short dental procedure. It is relatively painless with very little post-operative pain. The threads on the implant post allow for the bone to fill in and integrate. To facilitate this process the implant is re-covered with gum tissue and allowed to heal and integrate for nearly three months. The implant acts as the root for the tooth to provide solid and stable support for the crown that’s yet to be placed.

The next step in the dental procedure is to uncover the implant and place a healing cap to allow the gum tissue to heal. After a short period of healing, an impression is taken to fabricate a crown to fully restore the missing tooth. The crown is then cemented on top of the post, at which point you can resume normal eating activities.

Dental implants do require some special care, but that is easily managed when you follow the directions outlined by Dr. Kelly Peterson. During your regularly scheduled cleaning, special instruments are used to clean implants. While a dental implant cannot get a cavity, a condition known as peri-implantitis can occur. This is very similar to periodontal disease as the end result is dental implant loss and loss of bone structure. Be sure to floss the dental implant daily and run the floss under the implant crown as far as it can go to remove food and plaque. If you use any picks or small brushes to go in between your teeth, make sure they are plastic. Metal will scratch the implant making it more susceptible to infection. Be sure to keep your regular dental visits and cleanings to monitor the implant and help preserve your investment.

Energy Drinks and Your Teeth

When working out, studying late, or any time we feel like a bit of a boost would come in handy, energy drinks are a common go-to for many teens and adults. Energy drinks promise more focus and vitality, all wrapped up in clever names and eye-catching graphic designs. What they can’t promise, though, is a drink that’s good for your dental health.

Why? Because most energy drinks come with a high concentration of caffeine, acids, and/or sugar.

  • Caffeine Consequences

Before we talk about your dental health, a word about caffeine. Too much caffeine in a short period can leave us jittery, anxious, irritable, and sleepless. A cup of coffee has about 100 mg of caffeine, and some energy drinks have three to four times that much.

While the recommended daily limit for adults who aren’t pregnant is 400 mg of caffeine, doctors suggest a much lower limit for children and teenagers. Among other reasons, sleep is especially important for developing brains and bodies, and caffeine interferes with healthy sleep.

You might have to go online to find out how various energy drinks measure up and compare in terms of caffeine, but taking the time to check them out is well worth it. Now, back to your dental health!

  • Acid Attacks

Many energy drinks are very acidic. The tart flavors of energy drinks might please your palate, but they attack your enamel.

Our mouths are their healthiest when our saliva is neutral, balanced between acidic and basic on the pH scale. That’s somewhere around a 7 on a scale of 0-14, where lower numbers mean more acidic conditions. That burning feeling we get from acid reflux when stomach acids back up? That’s because stomach acid is a 1-2 on the pH scale.  Many energy drinks rate an eye-opening (and not in an energetic way!) 1.5-3.5 on the pH scale, which means your teeth are bathing in acids.

Even tooth enamel, the strongest substance in the body, can’t stand up to these acids, because acids break down the mineral structure of your enamel. If your teeth are more sensitive to heat and cold, if they appear darker as the white outer layer of enamel thins, if your dentist has discovered weak spots in your enamel—you might be experiencing demineralization. Once enamel is gone, it can’t be replaced.

  • Surplus Sugar

A well-known source of quick energy is sugar. A little more biology and chemistry here—sugar is a food which our bodies can break down quickly. And when sugar molecules break down, this breakdown releases energy—energy which we use to fuel essential bodily functions. This quick burst of sugar energy is why many energy drinks contain lots of added sugar. Some brands have more than 60 grams of sugar per serving—and 60 grams is almost 5 tablespoons!

We get all the sugar we require for our daily energy needs from the natural sugars found in fruits, dairy, even some vegetables and grains. With added sugars, we’re just adding empty calories, affecting the balance of our blood sugar and hormones, and increasing the risk of chronic health conditions—all while providing a convenient food source for the bacteria in plaque.

Because, just like our bodies find it easy to convert sugar to energy, the oral bacteria which cause cavities find it easy to convert sugar to acids. And just like the acids in foods, these acids attack the tooth’s mineral structure and break down its strength. Eventually, the weak spots in enamel grow larger and deeper and become cavities.

While most labels don’t let you know how much caffeine you’re getting, or the pH of the liquid inside, you can see how much sugar is being added to your diet with every can or bottle. Do look for added sugars before you choose your beverages.

If you have several energy drinks a day, a soft drink or two, plus the occasional sports drink—you’re bathing your teeth in acids and sugar all day long. With any beverage high in sugars and acids, limit your consumption. Rinse with water afterward, and don’t brush for about an hour to give your enamel time to remineralize after being exposed to the acids in your drink.

Better yet, choose healthier alternatives. Water is still the best way to hydrate. Try adding protein and carbs to your diet for more energy. Make sure you’re getting the vitamins you need with a balanced diet. If you see signs of enamel erosion, talk to Dr. Kelly Peterson. The temporary boost you get from a bottle of caffeine, acids, and sugar might seem tempting, but it can have long-lasting consequences for your dental health!

Treating Gum Disease with Antibiotics

Why does gum disease develop? Our mouths are home to bacteria, which form a film called plaque. Plaque sticks to the surfaces of our teeth, at the gumline, and can even grow below the gumline. And this bacterial growth leads to inflammation and gum disease.

When the disease progresses, the gums gradually pull away from the teeth leaving pockets which can be home to infection. Toxins can attack the bone structures and connective tissue, which support our teeth. Left untreated, periodontal disease can lead to serious infection and even tooth loss.

Because we are dealing with bacteria, it makes sense that antibiotics are one way to combat gum disease. Depending on the condition of your gums, we might suggest one of the following treatments:

  • Mouthwashes—there are mouthwashes available with a prescription that are stronger than over-the-counter antibiotic formulas, and can be used after brushing and flossing.
  • Topical Ointments—These ointments or gels are applied directly to the gums, most often used for mild forms of the disease.
  • Time-release Treatments—If there is severe inflammation in a pocket, we might place a biodegradable powder, chip, or gel containing antibiotics directly in the affected area. These minute methods release antibiotics over a period of time as they dissolve.
  • Pills and Capsules—For more serious periodontal disease, you could be prescribed an oral antibiotic. Take in pill or capsule form as recommended, and always finish the entire prescription.
